Your name, age, contact details (phone / LINE / email) and the initial symptom information you provide via the appointment or consultation form, solely for the purpose of providing medical services.

For appointments, consultation, treatment planning and follow-up, and contacting you back. Health information is kept strictly confidential and accessible only to the relevant medical staff.

3. Disclosure of information

We will not disclose or sell your information to third parties for marketing without your consent, except where required by law.

4. Your rights as the data subject

You have the right to access, correct, delete or restrict the use of your personal data, and to withdraw consent at any time, by contacting the clinic via the channels below.
